Addendum #1

Bidders are advised that the original Contract Documents for the referenced Contract have been amended as noted in this Addendum No. 1. This Addendum shall become part of the Contract Documents and is to be attached thereto.  Bidders shall acknowledge receipt of this Addendum on page P-11 of the Bid Documents.

Form of Proposal

The bidder shall remove pages FP-1 through FP-5 of the bid documents and replace them with the attached pages FP-1 ADD1 through FP-6 ADD1. (Specifically, the pay items have been broken out into formal and informal testing).

This Addendum does not change the date or time of receipt of bids.
